Our store is hosted on Hostgator.in and powered by Woo Commerce. They provide us with the online e-commerce platform that allows us to sell our products and services to you.

SECTION 20 – CONSENT FOR APPOINTMENT BOOKINGS

My care is directed by Qualified Mental Health Practitioners (MHP). I consent to services rendered and provided to me under the instructions and authorize Miind My Miind and its MHPs to use the telehealth practice platform for telecommunication for evaluating, testing, and diagnosing my condition and for the relevant therapy.

I understand that technical difficulties may occur before or during the tele-counselling / tele-psychiatry sessions and my appointment cannot be started or ended as intended at times. I accept that the professional can contact interactive sessions with video call; however, I am informed that the sessions can be conducted via regular voice communication if the technical requirements such as internet speed cannot be met.

I agree that my records can be kept for further evaluation, analysis and documentation and in all of these, my information will be kept private.

The fee for the evaluation does not guarantee that I will receive a prescription. If I do not qualify for a recommendation a refund will not be issued.

          The WEMWBS was developed and tested by a group of researchers at the Universities of Edinburgh and Warwick.
          The WEMWBS is used to measure subjective well-being in adults ≥16 years.
